      Insurance/ Billing - Physical therapy is a covered service with most insurance companies. Individual plans vary.


      It is always a good idea to know your Rights and Responsibilities. 
      When you are in need of physical therapy and your doctor has given you a prescription they may have referred you to a facility nearby. We want you to know it is your choice when deciding where you should go for physical therapy. You can simply call one of our offices and make an appointment today with one of our therapists.   We recommend you call your insurance company to obtain information about your coverage for physical therapy. Houghton physical therapy will assist but cannot be responsible in acquiring your Doctor Referrals or health care benefits.  HPTSC accepts most major insurance companies.  If your insurance company is not listed below, please do not hesitate to give us a call at 508-619-4169 x 1007.

      Check list for your billing and insurance paperwork.

      Referrals - If you have a HMO plan you will need to get a referral from your PCP. Insurance companies will not pay for your visits without a referral.
      Insurance card - Please bring your insurance card with you at your first visit so we can make a copy for complete billing information. If your insurance changes at any time during your care with us, please inform us as soon as possible to make adjustments.
      Workers comp/MVA - The date of your injury, claim #, adjusters name and telephone number is needed in order to schedule an appt and process your claim verification.
